NSW General

The following amendments have been made:

LPA_CA Costs Disclosure and Costs Agreement

• amended wording in "General Terms of Business" - Your Rights paragraph 5(e) changed from "substantial" to "significant"

LPA_CCA Costs Disclosure and Conditional Costs Agreement

• amended wording in "General Terms of Business" - Your Rights paragraph 4(e) changed from "substantial" to "significant"

FORM1 Standard Costs Disclosure Form for Clients (Uniform Law)

• correction to option numbers for Basis for Calculating Costs variable

VIC Magistrates' Court

The following forms have been amended in accordance with the Magistrates' Court (Chapter I Miscellaneous Amendments) Rules 2015 which come into effect on 1 February 2016:

MC FORM006A Affidavit of Service (blank)  

• Now includes option to include how the deponent identified/established the identity of the person served if required

MC FORM006A-1 Affidavit of Service of Complaint

• As with FORM006A above

MC FORM022A Application for Summary Order

Renamed to:

Summons for Application for Summary Judgment by Plaintiff

MC FORM022A-1 Application for Setting Aside/Variation of Summary Order

Renamed to:

Summons for Application for Setting Aside/Variation of Summary Order

The following new form has been added in accordance with the Magistrates' Court (Chapter I Miscellaneous Amendments) Rules 2015 which come into effect on 1 February 2016:

VIC VCAT

The following general VCAT forms have been amended:

VCAT AAA001 to VCAT AAA023 Minor coding amendment made to the header that does not affect the content of the forms

WA UCPR

The following changes have been made:

• Typo corrected

WAC SDC FORM39 to WAC SDC FORM43 Various Judgments

• Spacing issue resolved if costs to be taxed

AUS Fair Work Australia

The following form has been updated:

• Row heights amended from "Exactly" to "At Least" to allow them to expand if necessary
